(Translation: Destination Eurovision 2019. Call for candidates. Participate in Destination Eurovision to try and become the French representative for Eurovision 2019! (Application) form at the bottom of the page or at [link])

The form in question can be found here but those who wish to apply need to note of a few rules.
The Eurovision rules of not more than six people on stage, singing live and the song not having been publicly available before September 1st apply but also that singers need to be registered with SACEM (the French music association) or an assocation of equal status (which goes for both France and abroad) and the song may not be made available for commercial purposed before Destination Eurovision itself.
